Parent Transition Survey Information you provide will help with transition planning and instruction. Select options that communicate your thoughts and plans regarding your son or daughter’s transition. We will continue to update information so that it remains accurate for planning. Thanks very much!

Transition planning: Collaborative process to identify goals and plan for student success after graduation
PerformCare: If eligible after applying, services for students with I/DD are provided until age 21
Long Term Planning: Preparing for the future requires exploring residential options & financial planning
Guardianship: Protects students unable to legally make life decisions (finances, education, healthcare, etc)
SSI: Supplemental Security Income financial assistance (apply for eligibility at age 18 if appropriate)
Medicaid: Provided with SSI, Medicaid is important and is required for DDD services
DVRS: Division of Vocational Rehabilitation Services will provide pre-employment services for students 14+ if eligible. Services are also provided after graduation to help individuals accomplish employment goals.
DDD: Division of Developmental Disabilities provides a variety of services needed for daily living and employment
NJCAT: When applying for DDD, this survey assesses student needs. Results determine budget for services
Support Coordinator: Helps students and families effectively use their budget for DDD services
Postsecondary Options: College programs, Accommodations, Employment Training, Preparation and Planning
